No matter how bad an accident is, it can always be made worse. In the aftermath, you have to find out what happened, report the incident to one or more state or federal agencies, conduct an investigation and propose appropriate corrective actions.

A misstep at any of these stages can have far-reaching consequences. Privileged matters can be exposed to outside scrutiny. Use of a wrong word can make matters seem worse than they actually are. Improper speculation in your notifications or early reports can make it harder to ultimately arrive at the truth.
